This Dog's Health Package Contract ("Contract") is entered into this [Date] by and between [Your Name] ("Owner") and [Veterinarian's Name] ("Veterinarian").

WHEREAS, the Owner is the owner of a dog named [Dog's Name] ("Dog");

WHEREAS, the Veterinarian is a licensed veterinarian who provides veterinary services to the Dog;

WHEREAS, the Owner desires to purchase a health package for the Dog from the Veterinarian;

NOW, THEREFORE, in consideration of the mutual covenants and agreements contained herein, the parties agree as follows:

1. Services

The Veterinarian agrees to provide the following services to the Dog under this Contract:
Annual physical examinations
Vaccinations
Deworming
Flea and tick prevention
Heartworm prevention
Dental cleaning
Emergency care

2. Fees

The Owner agrees to pay the Veterinarian a monthly fee of [Amount] for the services provided under this Contract. The monthly fee is due on the first day of each month.

3. Term

This Contract shall commence on [Start Date] and shall continue for a period of one (1) year. The Contract shall automatically renew for additional one (1) year periods unless either party provides written notice of termination at least thirty (30) days prior to the end of the then-current term.
